Doramind-T Dorzolamide Hydrochloride & Timolol Maleate Eye Drops

Doramind-T is an ophthalmic eye-drop formulation containing Dorzolamide Hydrochloride and Timolol Maleate. It combines two medicines with complementary mechanisms of action to help reduce elevated intraocular pressure.

Dorzolamide is a carbonic anhydrase inhibitor, while Timolol is a beta-adrenergic receptor blocking agent. Together, these medicines reduce aqueous humor production through different pharmacological pathways, helping lower intraocular pressure.

Uses of Doramind-T Eye Drops

Doramind-T is used for the reduction of elevated intraocular pressure associated with:

• Open-angle glaucoma

• Ocular hypertension

It may be prescribed when additional intraocular-pressure reduction is required or when treatment with a beta-blocker alone does not provide adequate control. The fixed combination of Dorzolamide 2% and Timolol 0.5% has been clinically evaluated in patients with ocular hypertension and open-angle glaucoma.

Dorzolamide and Timolol for Glaucoma

Glaucoma is a group of eye conditions associated with progressive optic nerve damage. Elevated intraocular pressure is an important risk factor for glaucoma progression, and reducing intraocular pressure is a major component of glaucoma management.

Dorzolamide-Timolol eye drops provide a combination approach to intraocular-pressure management. Clinical studies have shown that the fixed combination can produce clinically meaningful reductions in intraocular pressure in patients with glaucoma and ocular hypertension.

Dorzolamide-Timolol for Ocular Hypertension

Ocular hypertension refers to elevated pressure inside the eye without the characteristic optic nerve damage associated with glaucoma.

Doramind-T may be used when reduction of elevated intraocular pressure is clinically indicated. The Dorzolamide component inhibits carbonic anhydrase and reduces aqueous humor formation, while Timolol reduces aqueous humor production through beta-adrenergic blockade.

The combined action of these two medicines helps provide additional intraocular-pressure reduction compared with either mechanism alone. 

How Dorzolamide Works

Dorzolamide is a topical carbonic anhydrase inhibitor.

Carbonic anhydrase contributes to the formation of aqueous humor within the eye. Inhibition of this enzyme decreases aqueous humor production, which helps reduce intraocular pressure.

Dorzolamide 2% has been clinically evaluated as an intraocular-pressure-lowering medicine in patients with glaucoma and ocular hypertension.

How Timolol Works

Timolol is a non-selective beta-adrenergic receptor blocker.

When administered ophthalmically, Timolol reduces aqueous humor production and consequently helps lower intraocular pressure.

The combination of Dorzolamide and Timolol therefore provides two complementary mechanisms for reducing intraocular pressure.

Clinical Evidence

A randomized clinical trial evaluated a fixed combination of Dorzolamide 2% and Timolol 0.5% in patients with bilateral ocular hypertension or open-angle glaucoma. The study compared the combination with the individual components and evaluated intraocular pressure over the treatment period. 

Clinical reviews have also reported that fixed-dose Dorzolamide-Timolol combination therapy is an effective and well-established topical approach for lowering intraocular pressure in glaucoma and ocular hypertension. 

A more recent randomized phase 3 study in Indian patients with primary open-angle glaucoma or ocular hypertension also included Dorzolamide 2%/Timolol 0.5% as a comparator and demonstrated clinically meaningful intraocular-pressure reductions with the fixed combination.

Because Timolol is a beta-blocker, ophthalmic Timolol can have systemic effects. The prescribing information includes important contraindications and precautions involving respiratory disease, certain cardiac conditions and hypersensitivity. 

Patients with asthma, a history of bronchial asthma, severe chronic obstructive pulmonary disease, sinus bradycardia, certain degrees of atrioventricular block, overt cardiac failure or cardiogenic shock should be assessed carefully before use according to the approved prescribing information. 

Patients should not discontinue prescribed glaucoma treatment without consulting their ophthalmologist.

When more than one topical ophthalmic medicine is prescribed, the medicines should generally be administered at an appropriate interval as directed by the healthcare professional.
